Forms
There are two complaint forms:

Making a complaint to your telecommunication company:
Use the Customer Complaint form (Word document). Download and print off the form. Then fill it in and send to your telecommunications company. -
Making a complaint to TDR:
Use the TDR Complaint form (.pdf file). Download and print off the form. Then fill it in and send (freepost) or fax to TDR.
You will need Adobe Reader to read the .pdf file. This is free software that you can download from the Adobe website.
